چکیده

The authors report a case of intraoperative reversal apixaban with andexanet alfa in patient supported venoarterial extracorporeal membrane oxygenation due to low- cardiac-output immediately after surgery for acute type A aortic dissection and massive transfusion administration procoagulants. In this patient, alfa's off-label use was not associated thrombotic complications despite being given during life support previous prothrombin complex concentrates. PRESCRIPTIONS OF direct oral anticoagulants are increasing constantly. It can be estimated that about 3% patients anticoagulated at some point will have undergo emergent procedures.1Garcia D Alexander JH Wallentin L et al.Management clinical outcomes treated vs warfarin undergoing procedures.Blood. 2014; 124: 3692-3698Crossref PubMed Scopus (114) Google Scholar Andexanet alfa, recombinant factor Xa without intrinsic activity, US Food Drug Administration–approved 2018 the inhibitors (eg, rivaroxaban) by high-affinity binding life-threatening bleeding. Principles for Management of Intraoperative Acute Type A Aortic Dissection.

Intraoperative Type A aortic dissection is a rare pathology with incidence of 0.06-0.32%. It is associated with a high mortality between 30-50%. Some associated risk factors, including hypertension, enlarged aorta, peripheral vascular disease, advanced age, atheroma, and high arterial pressure on cardiopulmonary bypass, have been identified. Modification of these risk factors could reduce the i...

Innominate artery reconstruction during emergent surgery for acute aortic dissection

Case description This is a case of acute aortic dissection with multiple tears, occluding innominate artery and causing brain and right hand malperfusion. Patient underwent successful emergent surgery in deep hypothermic circulatory arrest with antegrade cerebral protection for complete replacement of innominate artery and hemiarch. Complete innominate artery was replaced during cooling period ...

Andexanet Alfa for the Reversal of Factor Xa Inhibitor Activity.

BACKGROUND Bleeding is a complication of treatment with factor Xa inhibitors, but there are no specific agents for the reversal of the effects of these drugs. Andexanet is designed to reverse the anticoagulant effects of factor Xa inhibitors. METHODS Healthy older volunteers were given 5 mg of apixaban twice daily or 20 mg of rivaroxaban daily. For each factor Xa inhibitor, a two-part randomi...

Acute Type A Dissection and Acute Inferior MI Complicating Aortic Coarctation: A Case Report

Aortic coarctation is a rare but dangerous condition. Presence of aortic dissection in addition to coarctation is significantly rarer and more life-threatening. Here, we present a case of acute inferior MI due to aortic dissection complicating a native aortic coarctation. A 17-year-old boy was admitted to our emergency department due to severe chest pain and loss of consciousness. His ele...

Surgery for Aortic Disease Neurological Outcomes After Immediate Aortic Repair for Acute Type A Aortic Dissection Complicated by Coma

Background—Management of acute type A aortic dissection (AADA) complicated by coma remains controversial. We analyzed our experience in managing AADA complicated by coma to determine the relationship of duration of preoperative coma to postoperative neurological recovery. Methods and Results—Between September 2003 and October 2010, 181 patients with AADA were treated, including 27 presenting wi...
